A ‘made in Scarborough’ plan to advance equitable, integrated, and person-centred care  

Scarborough, ON – (July 9, 2025) 

The Scarborough Ontario Health Team (SOHT) is proud to announce the launch of its Strategic Plan 2025–2028, a renewed roadmap to guide the delivery of accessible, equitable, and integrated health and community services for the nearly 900,000 people who reside in, or use health services in, Scarborough. 

This plan reflects the voices of SOHT’s 40 partner organizations, clients/patients, caregivers, and community members. It builds on five years of collaboration and is grounded in a shared commitment to improving health outcomes and reducing disparities through coordinated, community-based care. 

Strategic Directions 

The plan outlines three interconnected strategic directions: 
1. Primary Care Transformation – Strengthening access to equitable, team-based primary care through leadership, co-design, and data-driven planning. 

2. Integrating Clinical Services and Supports – Advancing coordinated care for people with complex needs, with a focus on mental health, chronic conditions, and cross-sector collaboration. 

3. Evolving the SOHT – Building a mature, resilient organization with refreshed governance, performance measurement, and sustainable operations.
