Client and Family Advisory Committee
客戶和家庭諮詢委員會（CFAC）由組織成員和客戶／護理人員／家庭代表組成，他們與 SOHT 組織攜手合作，與執行團隊一起評估和設計執行計劃。
CFAC 有助於：提供與病人的經驗／夥伴關係／參與程度有關的實務現況；審視最可行方法並向 SOHT 提供切實的建議，提供可持續機制計劃，以確保跨 SOHT 合作夥伴網絡的協作病人夥伴關係，並共同設計一個可在 OHT各個層面使用的護理模式。
自委員會成立以來，他們建立了一個共同設計框架的基礎，讓 SOHT 合作夥伴組織中所有代表能就於改善措施成效的評核方法交流意見。他們一直致力支援委員會的成員，引入真正多元化的代表，就數個電子健康碼概念項目的策劃、監察及評估、檢討變革建議，以及參與 SOHT 的戰略撤退交流意見。

In Fall 2025, the Client Family Advisors (CFAC) began the process of creating a Patient Declaration of Values (DoV) for Scarborough Ontario Health Team (SOHT). We began reviewing the DoV from the Ministry of Health, and other Ontario Health Teams. We worked collaboratively with members of CFAC, both on-line and in person. To match with Scarborough values, we decided to follow the acronym EPIC CARE that has been part of SOHT from the beginning. We agreed to use the same format for the Patient Declaration of Values.

These are the values CFAC believes are important to our clients and their families. SOHT will promote this Patient Declaration of Values to our partners and ask that they let their clients and families know that they will do their best to uphold and meet all of these values.
