Scarborough Ontario Health Team Welcomes Primary Care Network Clinical Leads

Scarborough, ON — The Scarborough Ontario Health Team is pleased to announce that after a month-long recruitment process, Dr. Donna Vlahos and Dr. Rosemarie Lall have been successfully identified as the Clinical Leads of our Primary Care Network (PCN).  

The vision for Primary Care Network (PCN) is to connect, integrate, and support primary care providers within Ontario Health Teams to improve the delivery and coordination of care for patients. The objectives for PCN are to organize the local primary care sector in Ontario Health Team planning and provide a voice in OHT decision-making. The PCN will also serve as a vehicle to support OHTs in the implementation of local and provincial priorities. 

Drs. Lall and Vlahos will provide clinical leadership to SOHT through its Primary Care Network. The role focuses on improving clinical governance, supporting service transformation, and ensuring high-quality, client-centered care aligned with SOHT. These leadership positions are designed to reflect the voices of primary care providers and community members, ensuring a collaborative and person-centered approach to care. 

We look forward to their clinical leadership in advancing SOHT and Ontario Health’s priorities that focus on primary care transformation, integrated services, and inclusive governance.

About Primary Care Network 

PCNs connect, integrate, and support primary care providers to improve how care is delivered and coordinated for patients.  

The PCN will have the following five functions:  

  1. The PCN connects primary care within the OHT. 
  2. The PCN services as a vehicle for providing the local primary care sector’s voice in OHT decision-making. 
  3. The PCN supports OHT clinical change management and population health management approaches.  
  4. The PCN facilitated access to clinical and digital supports and improvement for primary care. 
  5. The PCN supports local primary care Health Human Resource planning within the OHT.
