Subject:  Interprofessional Primary Care Team Funding Round 2  

Dear Primary Care Leaders in Scarborough, 

The call for proposals by the Government of Ontario for Round 2 of the Interprofessional Primary Care Teams presents an exciting opportunity to expand access to team-based primary care in Scarborough communities.  

Ontario Health has noted that Ontario Health Teams and Primary Care Networks (PCN) will be responsible for leading local efforts to support family doctors, nurse practitioners and primary care teams to identify attachment gaps and coordinate and submit proposals. For Scarborough Ontario Health Team (SOHT)/Primary Care Network (PCN), we have been asked to submit three (3) distinct proposals for Round 2 of the Interprofessional Primary Care Team (IPCT) expansion. 

As there is significant interest in this opportunity from across primary care providers in Scarborough and to mitigate conflicts of interest, the SOHT’s Collaboration Council has endorsed the creation of an independent working group that will finalize and share a Decision-Making Framework that will be used to identify the three submissions for SOHT. Early next week, SOHT will share this decision-making framework and details of the approach with all SOHT partners.
