The Carefirst FHT is a multi-disciplinary team of physicians, nurses, social workers, dieticians, physiotherapist and medical receptionists. The team collaboratively delivers exceptional comprehensive primary care to our communities. Our spectrum of services covers diagnosis, treatment, wellness promotion and prevention programs. We have a robust Chronic Disease Management and Prevention program and are actively involved in many of the OHT’s primary care initiatives. Our partnerships across the community ensure our patients receive coordinated and high quality of care. As a community FHT we collaborate closely with Carefirst Seniors and Community Services Association and also have unique access to a team of on-site specialists including cardiology, endocrinology, nephrology, optometry, geriatrics medicine, obstetrics/gynaecology, chiropody and audiology.

East GTA Family Health Team is a group of leading and innovative primary care health professionals who work together to provide the highest quality care for patients and their families. The Family Health Team offers targeted programs and services, in multiple languages, to diagnose and treat acute and some chronic conditions. Comprehensive prevention and education programs to address health behaviours and conditions are also offered to help patients achieve and maintain good health.

A Nurse Practitioner-Led clinic (NPLC) in Ontario is an innovative primary health care delivery model funded by Ministry of Health and Long-Term Care, in which Nurse Practitioners (NPs) are the lead providers of care within an interprofessional health team.
Hong Fook Mental Health Association is the leading ethno-cultural community mental health agency with a consolidated culturally competent team serving Asian and other communities. With the support from our health collaborations and the inter-professional team from our affiliated agency HF Connecting Health Nurse-Practitioner Led Clinic, Hong Fook provides a continuum of integrated primary care and mental health services covering the spectrum from “promoting wellness” to “managing illness” in Asian communities.
The Inner City Family Health Team provides seamless multidisciplinary primary care to homeless and previously homeless individuals in Toronto, with a particular focus on current and former residents of Seaton House men’s shelter. It offers a clinical team located in the Scarborough Village Residence to provide residential support to older individuals, originally those from Seaton House. The Inner City Family Health Team addresses their health, housing, and social care needs while transitioning to community based living.
Integrated Rehab Professionals is the largest “therapist owned” provider of home care rehabilitation in Durham Region. IRP has Physiotherapists, Occupational Therapists, Social Workers, Dietitians and physiotherapy assistants who provide in-home assessment and treatment of patients in the community. Located in Pickering, Integrated Rehab Professionals provides rehabilitation services to clients in their homes in Durham Region and in Scarborough, Lindsay, Haliburton and Port Hope Areas.
Our clinic, Pickering Sports Medicine & Wellness Centre services all patients: WSIB, VAC, MVA, private patients and all injuries. It is an out-patient physiotherapy clinic that offers publicly funded (OHIP) physiotherapy services for eligible patients.

Mon Sheong Foundation is a charitable organization to promote Chinese culture and heritage through caring for the elderly and nurturing the young. Mon Sheong Foundation provides programs and services to the elderly and to the young in response to the needs of Canada’s culturally diverse communities. The Foundation is home to four long-term care centres, an adult day program, three Chinese school campuses, a youth group, two community and volunteer services centres, three independent living senior apartments, and private care, and charity care units.

The Scarborough Academic Family Health Team (SAFHT) is a non-profit corporation that consists of an integrated group of healthcare providers which includes family physicians, nurse practitioners, a clinical pharmacist, registered nurses, registered dietitians, a case manager, clinical social workers, and clinical chiropodists. SAFHT is affiliated with the Scarborough Family Health Organization and the Victoria Health Centre Family Health Organization. Its goals are to make a significant and lasting improvement in community health, to enhance access to primary care, to increase the capacity of individuals to improve their own health, and to decrease the use of local hospital emergency departments.
SCHC is an accredited comprehensive community health organization delivering a range of holistic health and wellness services from early years to end of life. We have been serving vulnerable Scarborough residents (newcomers, visible minorities, families living with poverty, homelessness and unemployment) experiencing health inequities for over 45 years.
SCHC addresses the physical, mental, social, financial and environmental aspects of health through 42 integrated and accessible programs for primary and mental health and disease prevention, community engagement and wellness support as well as assisted living, home support services, a food bank, senior’s wellness and youth leadership activities plus much more.
The Scarborough Family Physicians Network is the unified voice of primary care physicians in Scarborough. Through our leadership, we are actively working to improve Scarborough’s primary care physician and patient experience. This will ensure that patients have timely access to both specialist and allied heath care with services tailored to our Scarborough population. TAIBU Community Health Centre offers Black-identifying clients from throughout the Greater Toronto Area access to primary care, health promotion and disease prevention programs in a culturally affirming environment.
Tendercare Living Centre is a Long Term Care Facility which provides nursing, personal care and convalescent care for 254 residents in a five storey building. It has two retirement homes: McNicoll Manor which caters predominately to Chinese speaking residents from within the Greater Toronto Area and Moll Berczy Haus which offers security, comfort and happiness in Toronto’s only residence for German speaking seniors.

The Housing Help Centre (THHC) is a registered non-profit agency whose mandate is to help people access and maintain affordable and safe market rent accommodation. The housing help program works with youth, families, newcomers to Canada, and seniors who are homeless, under-housed, and/or at risk of losing their housing.
The Wexford Residence Inc. is a vibrant seniors residence in Scarborough that combines independent apartments, long-term care and a seniors’ community centre.
TransCare is an accredited Community Support organization which provides in-home support as well as community programs for seniors and caregivers. TransCare works in collaboration with local hospitals, Home and Community Care Support Services as well as health service providers to support seniors to live in the community.
Founded in 1925, VHA Home HealthCare (VHA) is a not-for-profit, charitable organization made up of over 3,000 diverse professionals-nurses, personal support workers, rehab therapists, home support workers, extreme cleaners, and office support staff. Together, we provide high quality care, with great heart, to vulnerable populations in the communities we serve. We provide service 24 hours a day, seven days a week to clients in their homes, in schools, in retirement homes and other locations in the community. Yee Hong is a not-for-profit organization serving seniors and caregivers of diverse cultural backgrounds and who are living in every setting, to maintain health, independence and dignity. Yee Hong provides a full continuum of the highest quality, culturally appropriate and person-centred care from four campuses of care in Scarborough, Markham and Mississauga. Yee Hong serves 805 long-term care residents, operates a 10-bed residential hospice, and provides community-based, in-home and virtual services to over 14,000 clients and caregivers, including active seniors groups, home support services, adult day programs, meals and transportation, and palliative care programs. Yee Hong builds organization and system capacity and integration through education, advisory services and research.
