It builds on five years of collaboration and is grounded in a shared commitment to improving health outcomes and reducing disparities through coordinated, community-based care.\u00a0<\/p>\n<!-- \/wp:paragraph --><!-- wp:paragraph -->\n<p><strong>Strategic Directions<\/strong>\u00a0<\/p>\n<!-- \/wp:paragraph --><!-- wp:paragraph -->\n<p>The plan outlines three interconnected strategic directions:\u00a0<br \/>1. Primary Care Transformation \u2013 Strengthening access to equitable, team-based primary care through leadership, co-design, and data-driven planning.\u00a0<\/p>\n<!-- \/wp:paragraph --><!-- wp:paragraph -->\n<p>2. Integrating Clinical Services and Supports \u2013 Advancing coordinated care for people with complex needs, with a focus on mental health, chronic conditions, and cross-sector collaboration.\u00a0<\/p>\n<!-- \/wp:paragraph --><!-- wp:paragraph -->\n<p>3.
