To coordinate our work, SOHT has six (6) main committees who drive the work of our Ontario Health Team (OHT) forward, generally focused around our main priorities: Driving System Integration, Developing Operational Excellence, and Keeping #ScarboroughStrong. The membership of all committees includes team members from partner organizations, primary care physicians, client and family advisors and SOHT team members.<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">\u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">SOHT is committed to consistent, transparent communication with our partners and community. As part of these efforts, here are some of the key priorities our 6 SOHT main committees are working on.<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">\u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><b><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Recovery and Sustainability Committee<\/span><\/b><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">This committee\u2019s aim is to develop pathways and initiatives that will more effectively coordinate the delivery of healthcare amongst health and community care providers in Scarborough to improve the health and wellbeing of Scarborough residents.<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Current priorities for this committee include:<\/span><\/p><p class=\"MsoListParagraphCxSpFirst\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Developing integrated care pathways for patients with Diabetes to help improve access to and integration of care between providers based on best practices<\/span><\/p><p class=\"MsoListParagraphCxSpMiddle\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Implementing preventative cancer screening events for eligible patients to receive mammograms, pap tests and colorectal screening in line with Ontario Health guidance <\/span><\/p><p class=\"MsoListParagraphCxSpLast\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Developing and implementing initiatives to proactively address expected seasonal increases in flu, COVID and RSV in priority target locations across Scarborough<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">\u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><b><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Quality and Performance Committee<\/span><\/b><\/p><p class=\"MsoNormal\" style=\"line-height: 106%\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">The Quality and Performance Committee (Q&amp;P) committee is focused on implementing quality improvement initiatives across SOHT to help address capacity challenges across the sector to help ensure Scarborough residents can receive the care they need in the most appropriate place for them. This work specifically includes:<\/span><\/p><p class=\"MsoListParagraphCxSpFirst\" style=\"text-indent: -18.0pt;line-height: 106%\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Helping patients who are considered Alternative Level of Care (ALC), which means they continue to need around-the-clock care but no longer require an intense level of acute, hospital-level care, to safely transition out of hospital with the care they need in the community to stay well<\/span><\/p><p class=\"MsoListParagraphCxSpMiddle\" style=\"text-indent: -18.0pt;line-height: 106%\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Provide patients experiencing mental health and addictions challenges with services in the community so they do not need to go the Emergency Department to receive care<\/span><\/p><p class=\"MsoListParagraphCxSpLast\" style=\"text-indent: -18.0pt;line-height: 106%\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Increase cancer screening for Scarborough residents by providing specialized training to providers designed to help ensure patients are comfortable accessing screening \u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><b><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Digital Health Committee<\/span><\/b><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">SOHT\u2019s Digital Health committee supports the use of digital health tools and assets across our OHT to improve care in Scarborough. This work is focused on using digital health tools and assets to:<\/span><\/p><p class=\"MsoListParagraphCxSpFirst\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Improve communication amongst healthcare providers across a patient\u2019s circle of care. Specifically, SOHT is currently involved in a pilot project to text a secure messaging solution to help facilitate communication between providers. <\/span><\/p><p class=\"MsoListParagraphCxSpMiddle\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Improve access to primary care through digital initiatives onboarding primary care providers onto Online Appointment Booking platforms<\/span><\/p><p class=\"MsoListParagraphCxSpLast\" style=\"margin-bottom: 0cm;text-indent: -18.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Improve seniors\u2019 access to health and social services in Scarborough. This work currently involves building awareness of Ontario\u2019s eServices Program to expand the usage of these services, as well as supporting the use of the <\/span><span lang=\"EN-US\"><a href=\"https:\/\/torontoservicedirectory.ca\/\"><span style=\"font-family: 'Arial',sans-serif\">Toronto Health and Social Services Directory<\/span><\/a><\/span><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">, an initiative with 7 other Toronto-area OHTs that was launched in Spring 2023.\u00a0 <\/span><\/p><p>\u00a0<\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\"><strong>Privacy Committee<\/strong><\/span><\/p><p>The purpose of SOHT\u2019s Privacy Committee is to develop and establish a Privacy Program for SOHT.\u00a0The responsibilities of the Privacy Committee are to:<strong>\u00a0<\/strong><\/p><ul><li>Establish\u00a0 the Privacy governance framework, policies, and procedures for SOHT<\/li><li>Guide the development of SOHT agreements pertaining to items such as information\/data sharing across the care continuum and other pertinent agreements that will aid in the transformation of care in Scarborough\u00a0<\/li><li>Establish the oversight and monitoring framework as well as the framework for the identification and mitigation of privacy risks, emerging risks, as well as compliance;<\/li><li>Establish the education, training and awareness program for systems that may be shared across the OHT partner organization\u00a0<\/li><\/ul><p>SOHT established a Data Sharing Agreement (DSA) to govern how personal health information is shared between signing SOHT members.\u00a0 You can find it <a href=\"https:\/\/ourhospital.sharepoint.com\/sites\/OHT\/Shared%20Documents\/Forms\/AllItems.aspx?FolderCTID=0x012000E1864A326627924F9C15E010D268BE79&amp;id=%2Fsites%2FOHT%2FShared%20Documents%2FSector%20Tables%20%26%20Committees%2FSOHT%20Committees%20%26%20Working%20Groups%2FPrivacy%2FPrivacy%20Agreements%2FFinal%20%2D%20DSA&amp;viewid=15eeef94%2D2305%2D4507%2D9f33%2Df711b9768613\">here.<\/a><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">SOHT\u2019s <b>Governance committee<\/b> is responsible for making sure SOHT has the appropriate governance structures in place to meet the changing needs and priorities of SOHT while aligning with direction provided by Ontario Health. This includes:<\/span><\/p><p class=\"MsoListParagraphCxSpFirst\" style=\"text-indent: -18.0pt;margin: 0cm 0cm 0cm 39.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">Evaluating the effectiveness of our governance structures in advancing integrated care in Scarborough.<\/span><b><\/b><\/p><p class=\"MsoListParagraphCxSpLast\" style=\"text-indent: -18.0pt;margin: 0cm 0cm 0cm 39.0pt\"><!-- [if !supportLists]--><span lang=\"EN-US\" style=\"font-family: Symbol\">\u00b7<span style=\"font-size: 7pt;line-height: normal;font-family: 'Times New Roman'\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span><!--[endif]--><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">\u00a0Evaluating the organization of different healthcare sectors within our governance structures to effectively promote collaboration across sectors. <\/span><b><\/b><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">\u00a0<\/span><\/p><p class=\"MsoNormal\" style=\"margin-bottom: 0cm\"><span lang=\"EN-US\" style=\"font-family: 'Arial',sans-serif\">SOHT\u2019s <b>Finance committee<\/b> is responsible for ensuring that funding provided to SOHT is effectively managed.
