une r\u00e9ponse collaborative \u00e0 la saison de la grippe, en analysant les donn\u00e9es pour identifier les quartiers \u00e0 haut risque o\u00f9 les partenaires pourraient offrir des cliniques de grippe suppl\u00e9mentaires.<\/li><li>Plusieurs centres d'\u00e9valuation COVID-19 ont \u00e9t\u00e9 mis en place pour offrir des services de d\u00e9pistage dans les zones \u00e0 haut risque, y compris des tests pour les r\u00e9sidents et le personnel de toutes les maisons de soins de longue dur\u00e9e, de plusieurs maisons de retraite, des r\u00e9sidences assist\u00e9es et des \u00e9tablissements de soins collectifs, ainsi que des cliniques de d\u00e9pistage communautaires dans les quartiers cibles.<\/li><li>PROTECH (Pandemic Rapid-response Optimization to Enhance Community-Resilience and Health) : un projet d'action communautaire d'intervention rapide qui est une ressource en ligne et un centre de formation, soutenu par le gouvernement du Canada).<\/li><\/ul><p style=\"font-size: 18px;font-style: normal;font-weight: 400\"><span style=\"font-weight: bold\">Pour notre population de ann\u00e9e un<\/span><\/p><ul><li>GAIN : 2 \u00e9quipes hospitali\u00e8res et 2 \u00e9quipes communautaires collaborant dans le cadre d'un mod\u00e8le de soins standardis\u00e9, offrant des services d'orientation, d'aiguillage et d'intervention aux personnes souffrant de fragilit\u00e9 li\u00e9e \u00e0 l'\u00e2ge.<\/li><li>PCCT : un partenariat intersectoriel comprenant les soins de longue dur\u00e9e, l'espoir et les fournisseurs de soins \u00e0 domicile et communautaires, qui s'appuie sur les services d'hospice communautaires existants pour inclure la navigation dans le syst\u00e8me palliatif avec des infirmi\u00e8res expertes, un service de garde 24 heures sur 24 et 7 jours sur 7, des consultations sur la gestion de la douleur et des sympt\u00f4mes, des conseils psychosociaux et spirituels, et le recrutement de m\u00e9decins palliatifs communautaires pour devenir la plaque tournante des soins palliatifs.<\/li><li>Programme \"Assess and Restore\" : pour la physioth\u00e9rapie et l'ergoth\u00e9rapie des personnes \u00e2g\u00e9es apr\u00e8s leur sortie de l'h\u00f4pital.<\/li><li>Partenariat de l'h\u00f4pital et de la communaut\u00e9 de soins aux personnes \u00e2g\u00e9es avec l'Universit\u00e9 McMaster pour une recherche sur les interventions communautaires dans le domaine du diab\u00e8te chez les personnes \u00e2g\u00e9es.<\/li><li>Fourniture de programmes de lutte contre la toxicomanie pour les adultes et les personnes \u00e2g\u00e9es, comprenant un logement avec services de soutien, la gestion de cas et le conseil, la consultation d'un psychiatre g\u00e9riatrique et l'\u00e9change de connaissances avec un groupe local.<\/li><li>Am\u00e9lioration de la prestation de services et des r\u00e9sultats pour les personnes souffrant de l\u00e9sions c\u00e9r\u00e9brales acquises (LCA) \u00e0 Scarborough.<\/li><li>Transformation de SCOPE : Am\u00e9lioration de SCOPE \u00e0 Scarborough pour renforcer le soutien interprofessionnel des fournisseurs de soins primaires par le biais d'un point d'acc\u00e8s unique.<\/li><li>Programme de partenariat de Lawrence East : Services de type \" hub \" dans les b\u00e2timents du TCHC, offrant aux r\u00e9sidents des services de gestion de cas, de soins infirmiers, d'aide au logement, de soutien par les travailleurs sociaux et de soutien par les pairs.<\/li><li>Mise en place de la ligne centrale d'intervention COVID-19 pour les services de soutien communautaire.<\/li><li>Les cliniques m\u00e9dicales ont \u00e9t\u00e9 remplac\u00e9es par des rendez-vous virtuels et t\u00e9l\u00e9phoniques, principalement pour les personnes sortant des urgences ou des unit\u00e9s d'hospitalisation, les personnes orient\u00e9es par la communaut\u00e9 (m\u00e9decins g\u00e9n\u00e9ralistes) et les foyers de soins de longue dur\u00e9e.<\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>OUR WORK Aligned with the mandates of Ontario Health and the Ministry of Health, the Scarborough Ontario Health Team (SOHT) is committed to delivering a comprehensive and coordinated continuum of care to the population it serves.
